Two Options To Order

1. Enjoy FastStart Phone Service

If you are already taking Deplin® or another maintenance medication, we'll contact your doctor to obtain a new prescription for you! Simply call 1-877-321-2652 and ask for FastStart service. Please have your ValueCardRx ID number, prescription information (including your doctor's name and phone number), shipping address and credit card information ready.

Fast Start customers may call 1-877-321-2652 for your prescription prices. (Illinois residents please add 1% sales tax. Louisiana residents will be charged sales tax, if applicable.)

2. Complete the Mail Service Order Form

We accept VISA, MasterCard, Discover or American Express. You can also pay by check or money order. Do not send cash.

Allow 10-14 days for delivery of your medication from the day you mail your order. Standard shipping is free. Overnight or second-day shipping is available for an additional charge. Expedited shipping only affects shipping time, not processing time of your order. On average, with expedited shipping, you will receive your medication within 7-12 days from the day you mail your order.

Note: This is a discount program and not an insurance plan. Discounts are available only at Caremark mail service. Caremark may obtain manufacturer rebates on your prescription drugs. These rebates may be retained by Caremark to enable us to offer you low prices on medications through the Mail Order Direct program.

Mail Service

The Mail Service program lets you purchase a 90-day supply of Deplin® and your other maintenance medications through CVS Caremark Mail Service Pharmacy for additional savings. Plus, enjoy free standard shipping and handling!

Quality Service and Safety
Each time you fill a prescription using your ValueCardRx Savings Program card, CVS Caremark checks your CVS Caremark personal drug profile (including drugs filled at local participating pharmacies) for possible drug interactions and other safety concerns. Vital information is included with every order to ensure the proper use of each drug. In addition, a pharmacist is available 24 hours a day, 7 days a week to answer any questions. Simply call the toll-free number listed on your prescription label.

Convenient Refills
You can request prescription refills 24 hours a day by calling the toll-free number on your prescription label or by going online to CVS Caremark. All you need is your prescription number and credit card number. It's that easy!

Ordering Is Easy!
Use FastStart Phone Service to place your Mail Service prescription order over the phone or complete the Mail Service Order Form and mail it to the address listed on the form.

This is NOT insurance. Discounts are only available at participating pharmacies. By using this card, you agree to pay the entire prescription cost less any applicable discount. Savings may vary by drug and by pharmacy. Savings are based on actual 2008 drug purchases for all drug discount card programs administered by CVS Caremark. The program administrator may obtain fees or rebates from manufacturers and/or pharmacies based on your prescription drug purchases. These fees or rebates may be retained by the program administrator or shared with you and/or your pharmacy. Prescription claims through this program will not be eligible for reimbursement through Medicaid, Medicare or any other government program. This program does not guarantee the quality of services or products offered by individual providers. To cancel your participation in this program simply discard your ID card and notify your pharmacy that you are no longer participating in this program. We do not sell your information. Call the participant toll-free number on the back of your ID card to file a complaint related to the availability of contracted discounts, services or other contracted obligations of this program.
